Orthognathic Surgery – Advanced Jaw Surgery to Improve Function, Bite, and Facial Harmony
Orthognathic surgery refers to a group of surgical procedures that reposition the upper jaw (maxilla), lower jaw (mandible), or both, to correct skeletal imbalances that cannot be treated with orthodontics alone.
Unlike braces, which move teeth within the jawbone, orthognathic surgery repositions the jawbones themselves in three dimensions. This allows correction of significant bite discrepancies, facial disharmony, and functional issues such as chewing difficulty, jaw pain, and airway obstruction.
At Dr Joy Dental Clinic, this treatment is always performed in collaboration between your orthodontist and oral and maxillofacial surgeon, using advanced digital planning and hospital-based surgical care under general anaesthesia.
Orthognathic Surgery Before and After
When orthodontics alone is not enough
Braces and aligners are highly effective but they have biological limitations. They can reposition teeth, but they cannot correct skeletal jaw discrepancies. Orthognathic surgery may be recommended when the underlying jaw relationship cannot be corrected orthodontically without compromising function or long-term stability.
Common indications include:
Skeletal Class III Malocclusion
Underbite caused by lower jaw prominence or upper jaw deficiency
Skeletal Class II Malocclusion
Overbite with recessed lower jaw, affecting facial balance and sometimes airway function
Skeletal Anterior Open Bite
Inability of the front teeth to meet due to jaw positioning.
Vertical Maxillary Excess
Long face syndrome, causing excessive gum show and increased lower facial height
Facial Asymmetry
Uneven jaw growth causing midline deviation or canting
Obstructive Sleep Apnoea
Jaw advancement surgery can significantly improve airway space in selected cases
Severe TMJ-Related Skeletal Imbalance
Contributing to chronic jaw discomfort or dysfunction

Our team-based approach at Dr Joy Dental Clinic
Orthognathic surgery is not a single-specialist procedure but a carefully coordinated process that brings together orthodontic and surgical expertise. At Dr Joy Dental Clinic Dubai, your case is collaboratively managed from the outset, with our orthodontist aligning and preparing your teeth for surgery while the oral and maxillofacial surgeon focuses on the precise repositioning of the jawbones.
Throughout every stage, a coordinated clinical team ensures seamless communication and continuity of care, so that orthodontic preparation and surgical planning are fully aligned resulting in greater precision, efficiency, and predictable long-term outcomes.
What to Expect During Treatment
Joint consultation and diagnosis
Your journey begins with a comprehensive evaluation involving both your orthodontist and surgeon. Diagnostic records may include photographs, digital scans, panoramic X-rays, cephalometric imaging, and CBCT 3D scans.
Digital surgical planning
Using advanced virtual surgical planning (VSP), your treatment is simulated in 3D. This allows precise prediction of jaw movements and facial changes, helping you understand expected outcomes before treatment begins.
Pre-surgical orthodontics (12–18 months)
Braces are used to align teeth within each jaw in preparation for surgery. This phase may temporarily make your bite appear more misaligned — a necessary step to allow accurate surgical correction later.
Surgical readiness confirmation
Once orthodontic preparation is complete, your orthodontist and surgeon jointly confirm readiness for surgery. Custom surgical guides are created, and your procedure is scheduled at a premium hospital.
Hospital-based surgery
All orthognathic procedures are performed at a fully accredited hospital in Dubai in collaboration with an experienced anaesthesia and surgical team. Oral and maxillofacial surgeon, who is also involved in your treatment planning, performs the surgery to ensure complete continuity of care from consultation through to recovery. The procedure is carried out under general anaesthesia and may involve repositioning of the upper jaw, lower jaw, or both depending on your case. Most patients are able to return home within 1–2 days after surgery.
Recovery and post-surgical orthodontics
Initial healing takes approximately 6 weeks, during which swelling gradually subsides and diet is modified. Once bone healing is confirmed, orthodontic treatment resumes to refine your bite and finalise results.
Retention and long-term stability
Once treatment is complete, retainers are provided and long-term follow-up is scheduled to ensure stability of your bite and jaw position.

What Our Patients Have to Say
I underwent orthognathic surgery at Dr. Joy Dental Clinic, and the entire journey was handled with exceptional care. From the initial consultation to post-surgery follow-ups, everything was well planned and clearly explained. The results have been life-changing both functionally and aesthetically.
I had jaw alignment issues for years, and finally decided to go ahead with orthognathic surgery. The team here was incredibly professional and supportive throughout the process. The planning, coordination, and outcome were excellent.
What stood out to me was the detailed planning before the surgery. The doctors explained every step, and I felt fully confident moving forward. The recovery was smoother than I expected, and the results are amazing.
Frequently Asked Questions
-
Am I a candidate for orthognathic surgery?
Candidacy depends on your skeletal jaw relationship, dental condition, facial proportions, and overall health. A joint consultation is required for accurate assessment.
-
Will my appearance change?
Yes. The procedure improves facial balance by repositioning the jaws. All changes are digitally planned in advance so outcomes are predictable and discussed clearly.
-
Will there be visible scars?
No. All incisions are made inside the mouth, leaving no visible external scars.
-
How long does treatment take?
Most cases take 18–24 months from start to finish, including orthodontics, surgery, and finishing phases.
-
Is the surgery painful?
The procedure is performed under general anaesthesia. Post-operative discomfort is typically manageable and controlled with medication.
-
Will my jaws be wired shut?
No. Modern surgery uses internal fixation (plates and screws). Elastics may be used temporarily, but the jaws are not wired shut.
-
Are plates permanent?
Yes, in most cases. Titanium plates remain in place permanently and do not interfere with daily life or medical imaging.
-
How long is recovery?
Initial recovery is 2–3 weeks for daily activities, with full healing progressing over several months.
-
Will insurance cover the procedure?
Coverage depends on your policy and medical necessity. Our team provides documentation to support insurance claims when applicable.
-
What if I don’t proceed after starting braces?
Orthodontic preparation is designed for surgical completion. If surgery is not performed, results may be compromised, which is why planning and consultation are critical before starting.
